2023 Game Changer Nomination Form

 

Thank you for taking time to nominate an individual or business for Barberton Community Foundation’s Game Changer award.

This award recognizes individuals and businesses who have had a positive impact on Barberton through their leadership, vision or commitment to this community.

Nominators should fill out this form as completely as possible. In-process applications cannot be saved, so please have answers ready. You may submit multiple nominations. If you have questions please call us at 330-745-5995.

This application closes on Friday, June 16 at 4:00pm.

Common Q&A

  • Your candidate does not have to be a Barberton resident. Nominees should be connected to Barberton through their work, volunteerism or residency.
  • Letters of support are highly encouraged and can be attached at the end of the form in the Supporting Letters/Documents section.
  • Think carefully about nominating a person vs. their business/organization. If you are nominating an individual, your application should reflect that individual’s accomplishments. If you nominate their business/organization, your application should reflect that business/organization’s accomplishments. 
  • Game Changers are selected in mid July, and recognized at the Foundation’s Annual Dinner in November.

*New 2023* Nominator Interviews

In an effort to improve decision making, the selection committee has chosen to hold interviews with nominators. Please indicate if you, the nominator, would be willing to be contacted about your nominee (in person or phone) by a committee member as part of the selection process. We estimate interviews will occur after applications close June 16 and before July 1. If you decide you do not want to be interviewed, the submitted application will be accepted as complete and will be considered at the selection meeting.
