Thank you for taking the time to nominate an individual or business for the Barberton Community Foundation Game Changer award. This award recognizes individuals and businesses who have had a positive impact on Barberton through their leadership, vision, or commitment to this community. Nominators should fill out this form as completely as possible. Incomplete applications cannot be saved, so please have answers ready. You may submit multiple nominations. If you have questions please call us at 330-745-5995. This application closes on Thursday, June 30 at 4:00pm.
